About the Company
Core Health Metrics IQ/Digital Health Information is a new and innovative system that provides dedicated care to the long-term care community, specifically to patients residing in their own home. We are an independently owned LTC pharmacy, telehealth physician practice (not open to the general public), and remote patient care facility. As a new ecosystem, we are working to expand our operations nationwide, one state at a time, and currently operate in Pennsylvania and New Jersey localities.
Overview
Our Care Team plays a crucial role in connecting patients within the community with our services and other resources. The team consists of two positions: Patient Care Liaison and Patient Care Navigator. These positions require an individual with a passion for public health, community engagement, and some experience/knowledge of the healthcare landscape. The Care Team Liaison serves as an office-based correspondent who outreaches to prospective patients. They also receive inbound calls from patients and Care Navigators. The Care Navigators serve as correspondents in the field going out to patient homes for prospective patient intake, home assessments, and device deployment/installation.
Responsibilities
- Outreaching to prospective patients
- Onboarding new clients and patients
- Educating patients and caregivers on our services and other resources
- Patient engagement in the home
- Managing client accounts
- Package materials and products manually
- Follow all applicable government regulations, including HIPAA
- Prescription delivery to patient homes (as needed)
- Attending community outreach events
- Other duties as assigned; job duties may vary depending on business needs
Qualifications
- High School diploma or equivalent required
- Previous experience in the Healthcare field (Pharmacy, Medical, Home Care, Home Health)
- Some knowledge and understanding of Medical and/or Pharmacy Insurance
- Some knowledge of medication (preferred)
- Some proficiency navigating computer applications/technology
- Good communication and interpersonal skills
- Good customer service skills
- Able to adapt in a changing environment
- Able to work during inclement weather
- Attention to detail
- Problem solver
- Able to physically perform light lifting
- Personable
- Comfortable engaging with the elderly community
- Reliable
- Valid driver's license and means of transportation (we provide a company vehicle for deliveries and field work)
- Bilingual is a plus but not required

- May sit or stand six (6) to eight (8) hours per day

Proposed Schedule
Mon-Fri 9:00am-5:00pm (40 hours per week)
Compensation & Benefits
- $18.00 - $24.00 per hour (negotiable based on experience)
