Job Overview:

We are seeking a highly motivated and dynamic individual for the role of Digital Health Navigator. In this role, you will be responsible for developing and implementing our digital health program, aimed at enhancing the overall health and wellness of our residents. You will also work closely with our team of health professionals and residents to ensure the successful implementation and adoption of our digital health initiatives.

Key Responsibilities:

Develop and implement a comprehensive digital health program that prioritizes the needs and preferences of our resident population
Collaborate with our team of health professionals to identify key areas for improvement in our digital health services
Conduct regular assessments and analysis of data to evaluate the effectiveness of our digital health program
Provide education and training to our residents on how to effectively use our digital health tools and resources
Serve as a liaison between our residents and healthcare providers to ensure seamless communication and coordination of care
Stay up-to-date with the latest advances in digital health technology and make recommendations for incorporating them into our program
Conduct community outreach and education initiatives to increase awareness and participation in our digital health program

Qualifications:

Bachelor's degree in healthcare, nursing, public health, or related field required
Minimum of 2 years of experience in a healthcare or health technology related role
Experience with digital health technology, such as telehealth, remote monitoring, and health tracking devices
Strong communication and interpersonal skills, with the ability to build relationships with residents, staff, and healthcare providers

Working Conditions:

This is a full-time position, with a Monday-Friday schedule and occasional evening or weekend requirements
Work will be primarily in an office setting, but may also require travel to resident homes or off-site locations for community outreach and education events
Must be able to physically navigate our community and facilities, including stairs and elevators, as needed
