Your Role on Our Team
We are seeking a highly accountable and tech-savvy Care Support Coordinator to become an operational anchor for our patient care team. We are looking for a professional who combines a deep sense of ownership with a compassionate, patient-first mindset to ensure our patients and providers are fully supported in a fast-paced, digitally-driven environment.
What You Will Own: Key Accountabilities
- Digital Patient Onboarding/Be a Friendly Guide: Independently manage the virtual patient intake process, leveraging our EMR and communication tools to ensure meticulous data collection and a seamless digital experience.
- Reliable Communication/Be the Go-To Person: Serve as the empathetic and reliable primary point of contact for patients, managing all inquiries (phone, email, portals) with a focus on providing compassionate, accurate, and timely resolutions.
- Proactive Scheduling: Take full ownership of complex scheduling for sleep studies, telehealth consultations, and follow-ups, proactively identifying and resolving conflicts to maintain an efficient clinic flow.
- Data Integrity: Leverage your tech skills to expertly navigate our Electronic Medical Record system, ensuring all patient documentation, referrals, and records are precise, secure, and up-to-date.
- Follow-Through: Diligently manage the end-to-end patient cases, tracking every step to prevent delays in patient care. Your follow-through is critical to our patients getting the testing and treatment they need.
- Seamless Clinical Collaboration: Provide rock-solid administrative support to our providers, anticipating their needs and ensuring they have the information required to deliver outstanding care.

- A Genuine Patient-First Mindset: You approach every interaction with empathy, patience, and a commitment to providing an exceptional and supportive patient experience. You are a natural problem-solver who advocates for patient needs.
- Tech-Savvy & Resourceful: You aren't just 'good with computers'; you learn new software (like EMRs, scheduling tools, and communication platforms) quickly and intuitively. You are comfortable troubleshooting common tech issues and thrive in a paperless, digitally-focused environment.
- Highly Accountable & Dependable: You take initiative, follow through on all commitments and understand the critical impact of your work on patient outcomes.
- Proven Experience: You have a minimum of 2 years of experience in a medical office or healthcare coordination role where you were relied upon for your accuracy and efficiency.
- Exceptional Organizer: You have a natural ability to manage multiple priorities, track details, and meet deadlines in a dynamic setting.
- Education: A Bachelor’s Degree is preferred, but we value equivalent professional experience, especially in a relevant healthcare setting.
- Asset: Bilingual proficiency in English and Spanish (or proficiency in other languages) is a plus.
- Work Authorization: You must have unrestricted work authorization in the U.S.

- Digital Presence: You are expected to be fully available, punctual, and actively engaged on-camera during scheduled hours to provide real-time, collaborative support.
- Technical Foundation: You must have a reliable, high-speed broadband internet connection.
- Location: Colorado is preferred. Or reside within the Mountain Time Zone.
