As a Digital Care Manager in our young, dynamic company, you will play a pivotal role in providing continuous support to patients, ensuring their health is proactively optimized. You’ll collaborate with patients, families, and healthcare providers to develop and implement comprehensive care plans. Key Responsibilities:

Care Plan Development and Implementation: Collaborate in the creation, implementation, and ongoing refinement of personalized care plans that align with CMS guidelines, empowering patients to better manage their chronic conditions.
Patient Engagement and Education: Engage with patients through phone consultations, and digital communication to provide education about their health conditions, treatment plans, and available community resources. Your guidance will help patients feel empowered to take control of their health.
Coordination of Services: Be the key link in facilitating referrals and scheduling for essential medical services, such as specialist visits, diagnostic tests, and therapeutic services, ensuring patients receive timely and necessary care.
Medication Management: Oversee patients' medication regimens, ensuring adherence and communicating potential side effects or drug interactions, while providing regular support to optimize their medication management.
Health Monitoring: Monitor patient health through continuous assessments, adjusting care plans as needed and collaborating with healthcare teams to respond swiftly to changes in patient status.
Documentation and Compliance: Maintain accurate and comprehensive patient records while adhering to all CMS documentation requirements and privacy regulations, ensuring the integrity of every step in the patient care process.
Multidisciplinary Collaboration: Work in synergy with a diverse team of physicians, nurses, social workers, pharmacists, and other healthcare professionals to deliver a coordinated, patient-centered care approach.
Outcome and Performance Tracking: Track the effectiveness of care plans by measuring improvements in patient outcomes and satisfaction, as well as reductions in hospital readmissions and health complications.

Qualifications:

Education: Medical Assistant certification preferred.
Experience: Minimum of 1 year in healthcare, with a strong preference for experience in chronic disease management or case management. Foreign medical grads preferred.
Skills: Deep understanding of the healthcare system and CMS regulations. Strong interpersonal communication skills and organizational abilities to efficiently manage multiple patients and priorities.
Languages: English, Medical, and conversational fluent. Spanish, Medical, and conversationally fluent. Bilingual preferred.
