Position: Community Outreach/Digital Health Worker
Covenant House, Inc (CHI) is a Federally Qualified Health Center (FQHC) that was established in 1964. CHI is located in Germantown and covers the Northwest section of Philadelphia with a primary focus on the provision of primary care services in medically underserved urban communities. CHI seeks to offer patient-led healthcare that welcomes and strengthens the Philadelphia community by integrating body, mind, and spirit.
Covenant House provides healthcare services within community settings (i.e., social service agencies, schools, and community centers) in addition to their primary health facilities to make services accessible to the community. To support their expanded healthcare services, CHI's National Health Corps Community Outreach/Digital Health Worker will provide critical prevention and intervention services through outreach, community education, and resource support to underserved populations.
With their new effort to develop school-based wellness centers, the Community Outreach/Digital Health Worker will serve partially within a local High School and will be vital in reaching students, parents, and the community.
Major Duties and Responsibilities
Cardiac Health
Conduct community outreach, develop/administer surveys, and conduct patient interviews to identify persons at risk of cardiac health issues at local venues
Create and distribute health-related print materials to the community as a follow-up to Covenant House's annual needs assessment
Identify digital health resources for constituents
Connect constituents to individualize digital health resources
Link constituents to community resources that will mitigate social determinants of health issues
Monitor and track constituent adherence to medically prescribed services via phone or telehealth
School-Based Wellness
Create outreach materials for school distribution
Recruit students to participate in Wellness Center services (i.e., physicals, workshops, counseling)
Serve as a sounding board for students
Connect students with external services as approved by the supervisor
Support Covenant staff in the administrative delivery of services.

Knowledge Required for the Position
Knowledge of AmeriCorps/Health Corps member requirements
Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.
Supervision
The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.
The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required
Review
The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.
Guidelines
Written and oral guides provide specific instructions for doing service
Most instructions are easily memorized and require little interpretation
Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files of previous projects
Guidelines are generally applicable, but the member independently makes adaptations in dealing with problems and unusual situations
Complexity
The facts or conditions of the service position are clear-cut, predictable, and apply directly to the assignment
